How Do I Reconstitute a Lyophilized Vial
Reconstituting a powder-filled (lyophilized) vial requires sterile technique to maintain product integrity. Always reference your specific leaflet or product guidelines for the exact amount of bacteriostatic (BAC) water required, as the volume dictates the final medication concentration.
Supplies Needed
Your powder vial
Bacteriostatic water (BAC water)
Sterile mixing syringe (e.g., 3ml syringe)
Sterile transfer needle (if your syringe isn't pre-attached)
70% isopropyl alcohol swabs
A designated sharps disposal container
Step-by-Step Instructions
1. Prepare Your Workspace
Wash your hands thoroughly with soap and warm water for at least 20 seconds. Clean your flat preparation surface with an alcohol swab or disinfectant.
2. Sanitize the Vials
Remove the protective plastic caps from both the powder vial and the BAC water vial. Vigorously scrub the gray rubber stoppers on top of both vials with fresh alcohol swabs and let them air dry completely.
3. Draw the Bacteriostatic Water
Unpack your sterile syringe and needle. Pull back the plunger to draw an amount of air equal to the prescribed volume of water you need to add. Insert the needle through the stopper of the BAC water vial and inject the air (this equalizes pressure and makes drawing the liquid easier). Invert the vial and withdraw the exact required volume of BAC water.
4. Transfer to the Powder Vial
Insert the needle through the center of the rubber stopper on the powder vial, typically at a 45-degree angle. Slowly press the plunger to transfer the BAC water. Aim the liquid stream toward the inner glass wallrather than squirting it directly onto the powder to avoid foaming.
5. Mix and Dissolve
Once all the liquid is transferred, remove the syringe and dispose of it safely in your sharps container. Gently roll or swirl the vial between your palms. Never shake aggressively, as vigorous agitation can damage or denature sensitive compounds. Continue gentle swirling until the powder is completely dissolved and the solution is clear.
6. Storage Requirements
Most reconstituted solutions must be stored in a household refrigerator (between 36°F and 46°F or 2°C to 8°C). Do not freeze. Bacteriostatic water contains benzyl alcohol to prevent bacterial growth, but it generally has a maximum shelf-life of 28 to 30 days after opening. Always label the vial with the date you mixed it.
